Decision support, not a substitute for professional judgment
Clinical Capture provides suggested diagnosis and procedure codes and documentation guidance to assist qualified professionals. It does not assign final codes, submit claims, or replace a certified coder, biller, or the treating physician's judgment. All output must be independently reviewed and verified against current CPT®, ICD-10-CM, the CMS Physician Fee Schedule, NCCI edits, and applicable payer policy before any claim is submitted. You are solely responsible for codes you submit.
Not a medical device
Clinical Capture is an administrative billing-and-coding support tool. It does not diagnose, treat, or make patient-care recommendations, and is not intended to be a medical device.
De-identified use
You agree to remove patient identifiers before entering operative notes and to use the service only with de-identified information, unless and until a separate agreement governing identifiable data is in place.
Acceptable use
You agree not to misuse the service, attempt to circumvent access controls, or use it to support fraudulent or non-compliant billing (e.g., coding work that was not performed). The tool is designed to support accurate, complete coding of work actually performed — not to inflate reimbursement.
Availability and changes
The service is provided on an “as is” basis during its current phase. Features and availability may change. We may update these terms; continued use constitutes acceptance.
Limitation of liability
To the maximum extent permitted by law, Clinical Capture and its founder are not liable for claims, denials, audits, or losses arising from use of the output. Verification by a qualified professional is a required step, not an optional one.
These terms are provided for transparency and are not legal advice. An organization adopting Clinical Capture should review them with counsel and may require a separate written agreement.
