EVAR and Open AAA Repair: Coding the Aortic Aneurysm Case
Abdominal aortic aneurysm repair is coded very differently depending on approach, and the endovascular family is deliberately comprehensive.
EVAR is a bundled code
- 34701 / 34702 — EVAR, infrarenal aorto-aortic (non-ruptured / ruptured)
- 34703–34706 — aorto-uni-iliac and aorto-bi-iliac configurations
These codes include the catheterization of the aorta and access vessels, the device deployment, and the intraprocedural imaging (the radiologic supervision and interpretation is no longer separate). Proximal/distal extensions and additional components are add-ons. Fenestrated repairs use their own family (34841–34848).
Open repair
- 35081 — open repair, infrarenal aneurysm
- 35102 — open repair, aneurysm involving the iliac vessels
Open repair codes select by the extent of aorta/iliac involvement; the graft is included. Separate diagnostic angiography applies only when a true diagnostic study is documented.
Clinical Capture selects the EVAR configuration or open code the note describes and keeps the bundled catheterization and imaging off the claim.
